Changes relative to upstream:
Renamed to teloa-skill-creator and rewrote the description for Teloa.
Removed the agents/openai.yaml guidance; agents/openai.yaml, references/openai_yaml.md and the three Python scripts under scripts/ are not included.
Replaced the original initialize and validate steps with the Teloa creation flow: check for duplicates, then save a draft for confirmation.
Replaced mentions of Codex with a neutral "the agent".
